Affidavit That Power of Attorney (POA) is in Full Force

Before submitting your Power of Attorney documents to NYCERS, check the date of the principal’s signature. If the principal signed more than one year ago, the agent must complete an Affidavit that Power of Attorney is in Full Force and submit it with the original POA documents.
In some cases, NYCERS may send a written request to the agent/s to reaffirm the validity of a POA that has already been approved to ensure the security of its member’s account.
