The Customer Service Center is easily accessible from the A, C, F and R lines (Jay Street/Metro Tech), and the 2, 3, 4 and 5 lines (Borough Hall).
NYCERS does not provide Notary services. NYCERS will waive the Notary requirement on most forms if you provide a current U.S. Passport, a valid Driver’s License or a Work ID issued by any NYC agency. Notable exceptions are Power of Attorney and Claimant Statements for survivor benefits; these forms must be notarized.
To help Customer Service respond to your inquiry when you visit,
please bring the following:

We encourage all applications that are time sensitive to be hand delivered to
our Customer Service Center. You take considerable risk of undocumented or lost
applications when mailing such documents.
